Jowls

Jowls vs Double Chin: How to Tell the Difference and What Helps

Jowls and a double chin are two of the most common concerns people raise when they start noticing changes to their lower face and neck, and the two are often confused with one another. While they can certainly overlap, they are caused by different things and often respond best to different treatments. Understanding which one you actually have is the first step to choosing the right solution.

In this guide, Dr Leonardo Fasano explains how to tell jowls and a double chin apart, what causes each, and which treatments at Health Excel’s London and Birmingham clinics tend to work best for each concern.

What Are Jowls?

Jowls develop when the skin and soft tissue along the jawline lose firmness and begin to sag downward, creating a loose, drooping appearance below the corners of the mouth and along the jaw. This is primarily a skin and structural support issue, caused by a natural decline in collagen and elastin, combined with gravity acting on the tissue over time.

Jowls tend to make the jawline look less defined and can give the face a tired or heavier appearance, even in people who are not carrying excess weight.

What Is a Double Chin?

A double chin, medically known as submental fullness, is primarily caused by an accumulation of fat beneath the chin, sometimes combined with skin laxity. Unlike jowls, a double chin is often present regardless of age and can affect people who are otherwise a healthy weight, particularly if there is a genetic tendency to store fat in this area.

A double chin creates fullness directly under the chin, whereas jowls create sagging along the sides of the jaw. The two can certainly occur together, particularly as we get older and experience both fat accumulation and loss of skin elasticity in the lower face.

How to Tell Them Apart

Location

Jowls sit along the jawline itself, creating a sagging or drooping contour on either side of the chin. A double chin sits centrally, directly beneath the chin, and can extend down towards the neck.

Texture

Jowls typically feel loose and are made up primarily of lax skin and soft tissue. A double chin usually has a softer, fuller feel, as it is largely made up of fat.

What Happens When You Tilt Your Head Back

A useful way to distinguish the two is to tilt your head back and look in the mirror. If the fullness beneath your chin reduces significantly, this points towards fat as the primary cause, consistent with a double chin. If the sagging along your jawline remains largely unchanged, this points towards jowls, which are driven by skin laxity rather than fat volume.

Treatment Options for Jowls

Because jowls are primarily a skin laxity issue, treatments that focus on tightening and firming the skin tend to be most effective. At Health Excel, options include:

  • Renuvion skin tightening: uses helium plasma and radiofrequency energy to tighten the skin along the jawline with minimal downtime. Find out more on our Renuvion skin tightening page.
  • Dermal fillers: can help restore lost volume in the mid-face and jaw area, which indirectly improves the appearance of jowls by re-establishing definition. Learn more on our dermal fillers page.
  • Skin boosters such as Profhilo or polynucleotide treatment: improve overall skin quality and firmness over time, which can soften the appearance of early jowls.

Treatment Options for a Double Chin

Because a double chin is primarily a fat issue, treatments that reduce fat volume tend to work best. These include:

  • VASER liposuction: can precisely remove excess fat from beneath the chin, creating a sharper jawline profile.
  • Aqualyx fat-dissolving injections: a non-surgical option that breaks down stubborn fat deposits over a series of sessions.
  • Renuvion skin tightening: often used alongside fat removal to tighten any residual loose skin once the fat has been reduced.

When You Have Both

Many patients have a combination of jowls and a double chin, particularly from their forties onwards. In these cases, a combined approach, addressing both fat reduction and skin tightening, usually delivers the most natural and complete improvement. Dr Fasano will assess your individual anatomy during consultation and recommend a tailored plan rather than a one-size-fits-all treatment.

Can Lifestyle Changes Help?

For a double chin, maintaining a stable, healthy weight can help reduce the amount of fat stored beneath the chin, and posture exercises are sometimes suggested, though the evidence for their effectiveness is limited. For jowls, there is little that lifestyle changes alone can do to reverse the loss of skin elasticity that comes with ageing, since this is a structural change rather than one driven primarily by weight. Good skincare, sun protection and staying well hydrated can help support skin quality generally, but they will not meaningfully correct established jowls or a double chin once they have developed. This is why most people who are bothered by either concern eventually look to a clinical treatment for a more noticeable, lasting result.
