Fat Transfer

Fat Transfer to Face: Natural Volume Restoration and Rejuvenation

As we age, one of the most noticeable changes to our face is the gradual loss of volume. Cheeks that were once full begin to flatten, hollows appear beneath the eyes, and the temples can start to look sunken. Many people seek a solution that looks natural, lasts a long time, and avoids synthetic materials. Fat transfer to the face offers exactly that: a procedure that uses your own body fat to restore youthful contours and rejuvenate your appearance.

What Is Fat Transfer to the Face?

Fat transfer to the face, also known as facial fat grafting or lipofilling, is a surgical procedure in which fat is removed from one area of your body and injected into targeted areas of your face. Because the fat comes from your own body, there is no risk of allergic reaction or rejection, making it a safe and biocompatible option for facial rejuvenation.

The procedure is performed under anaesthetic and is typically combined with liposuction of the donor site, which is commonly the abdomen, flanks, or inner thighs. At Health Excel, Dr Leonardo Fasano uses advanced harvesting techniques to preserve the integrity of the fat cells, which is critical to achieving long-lasting results.

How Does Facial Fat Transfer Work?

The procedure involves three key stages, each of which must be carried out with precision to ensure the best possible outcome.

Stage One – Fat Harvesting

Fat is removed from the donor area using a gentle liposuction technique. Dr Fasano uses VASER liposuction, which uses ultrasound energy to loosen fat cells before removal. This minimally invasive approach helps to preserve the viability of the fat cells, improving their survival rate once transferred.

Stage Two – Purification

Once harvested, the fat is carefully processed to remove excess fluid, damaged cells, and impurities. Only the healthiest, most viable fat cells are selected for injection. This purification step is vital to maximising how much of the transferred fat survives and integrates into the facial tissue.

Stage Three – Reinjection

The purified fat is then carefully injected into the target areas of the face using fine cannulas. Dr Fasano places the fat in small, controlled amounts at multiple tissue depths to encourage good blood supply and maximise retention. This layered technique also helps to achieve a smooth, natural-looking result rather than an overfilled appearance.

Which Areas of the Face Can Be Treated?

Fat transfer to the face is highly versatile and can be used to address a wide range of concerns. The most commonly treated areas include:

  • Cheeks and mid-face – restoring volume lost through ageing to lift and define the cheekbones
  • Under-eye hollows (tear troughs) – reducing the dark, sunken appearance beneath the eyes
  • Temples – filling in hollow temples that can make the face look gaunt or aged
  • Nasolabial folds – softening the lines that run from the nose to the corners of the mouth
  • Lips and lip borders – adding subtle fullness and definition to thinning lips
  • Jawline and chin – improving definition and balance in the lower face

During your initial consultation, Dr Fasano will assess your facial anatomy and discuss which areas would benefit most from volume restoration, taking into account your overall facial proportions and aesthetic goals.

What Results Can You Expect?

Fat transfer to the face produces natural-looking results that blend seamlessly with your existing features. Because the transferred fat is living tissue, it integrates with the surrounding area over time and responds to changes in your body in the same way as native fat.

Most patients notice an immediate improvement in volume following the procedure, though there will be some swelling in the initial weeks. As the swelling subsides and the fat settles, the final result becomes visible over a period of two to three months. It is normal for some of the transferred fat to be reabsorbed by the body during this time, which is why Dr Fasano carefully accounts for this when determining how much fat to transfer.

How Long Does Fat Transfer to the Face Last?

One of the greatest advantages of facial fat grafting compared to dermal fillers is its longevity. Once the transferred fat has established a blood supply and integrated into the tissue, typically within three to six months, the results can be permanent.

The fat that survives the initial period tends to remain stable for many years. However, it is important to understand that the face will continue to age naturally over time, so whilst the volume restored through fat transfer is long-lasting, you may wish to consider further treatments in the future to maintain your results.

Maintaining a stable weight after the procedure also helps to preserve the outcome. Significant weight loss after surgery can reduce facial volume, whilst significant weight gain may cause the transferred fat to expand.

Recovery After Facial Fat Transfer

Recovery from fat transfer to the face is generally straightforward, though some swelling and bruising in both the face and the donor site are to be expected in the first one to two weeks. Most patients feel comfortable returning to desk-based work within seven to ten days, though strenuous exercise should be avoided for four to six weeks.

At Health Excel, we place great importance on the quality of your recovery. Our dedicated post-operative treatment programme includes lymphatic drainage massage and compression support to minimise swelling and promote healing in both the donor and treatment areas.

Is Facial Fat Transfer Right for You?

Fat transfer to the face is well suited to patients who have experienced volume loss due to ageing, weight loss, or genetic predisposition. It is particularly popular amongst those who prefer a natural alternative to synthetic fillers or implants. Good candidates are generally in good overall health, are non-smokers or willing to stop smoking prior to surgery, and have realistic expectations about what the procedure can achieve.

Patients must also have a sufficient supply of donor fat available. If you have very low body fat, this may limit the amount of material available for transfer, and Dr Fasano will discuss whether this is likely to be a consideration during your consultation.
